This document informs you about the data I will be collecting from you and what that data will be used for.
Name and age – this basic information is used to identify you.
Address, email address, and phone number – this information is used to contact you.
Emergency contact and medical professional’s details – this information is only used if there is an
emergency or I need to contact your doctor.
Session notes – I will keep brief notes of your session(s) with me, which I will store securely.
All the data you share with me will be kept completely confidential unless:
1. I have been subpoenaed by a court of law to use your data to provide evidence.
2. It is clear that you or someone you know is at risk of harm or harming others.
I may also discuss your case with colleagues, but, in those circumstances, I would not use your name
or any other details that would make you identifiable.
Hardcopies of your data will be stored in a locked filing cabinet. Any data stored on my computer will be password-protected and kept private by using your initials in place of your full name. As such, only I will have access to your information.
I will keep your data, including your session notes, for as long as I am required to by my insurance company, after which time I will dispose of it. To dispose of your data, I will destroy all hard copy documents containing your personal information and delete all your data that I have stored electronically.
